PAYMENT POLICIES
Payment is expected when
services are rendered unless arrangements have been made in advance.
Dr. Bednar's staff will complete insurance forms and submit claims as
a courtesy to patients; any balance not paid or refused by the insurance
company is the responsibility of the patient or account gaurantor.
The dental office staff will also estimate insurance coverage and let
the patient know, in advance, the estimated coverage. Please understand,
this is an estimate of benefits, the final determination will be made
by your insurance carrier.

Our office accepts cash, personal checks,credit cards and various insurance plans.
Our office now offers CareCredit, a dental credit card with 3,6, and 12 month no interest payment plans or 24,36 and 48 month plans with 9.9% A.P.R.

INSURANCE INFORMATION
- Your insurance plan is a contract between
your employer and the insurance carrier. Limitations of this plan may
apply to some services. It is the responsibility of each patient to
learn the limitations of their dental plan.
- You agree that the charges you incur are your responsibility regardless of what your insurance company pays.
